Job Description:
A company is looking for a Care Coordinator II.
Key Responsibilities
Make 50-75 outbound calls daily to enroll CMS members into care
management
Establish contact information for CMS members and connect with
providers as needed
Document calls and submit daily production reports while managing a
caseload of 300 members
Required Qualifications
High school diploma or GED
2+ years of healthcare experience with high volume outbound and
inbound calls
Experience handling 50-75+ calls daily
Proficient in computer skills, including Excel and working with
multiple systems
Must have a private workspace, high-speed internet, and a
smartphone for remote work
